As Band 6 Physiotherapist, you will play a critical role in the Discharge to Assess multidisciplinary team. We will consider applications for people seeking full and part-time posts and flexible hours. Work with the wider MDT to provide specialist Physiotherapy assessments and interventions. Provide a specialist, high-quality physiotherapy service within DTA. Provide specialist physiotherapy assessment, diagnosis, treatment, and education to individuals who come through the Discharge to Assess process who may have a range of conditions and presentations. Provide advice, signposting, guidance, and information to health and social care professionals, patients, carers, relatives, or other non-professional contacts. Be an integral member of the MDT providing education, training, and dissemination of information to all healthcare professionals. Be an autonomous practitioner, holding a caseload and working without direct supervision. Continue own professional development in line with HCPC regulations. Person Specification
Experience
Experience of working flexibly within a MDT. Clinical experience in health or social care / experience in PT approaches and treatment planning. Experience of multi-pathology assessment / home and community assessment. Experience of effective caseload management and discharge planning. Experience of management, leadership, and supervisory skills. Ability to work autonomously and set own priorities. Documented evidence of CPD. Experience of working with staff from other agencies. Experience of supervision of junior staff and/or assistants. Experience of written risk assessment in a variety of settings. Experience of standard prescription wheelchairs / static seating and pressure care. Experience of posture and positioning. Knowledge and use of complex manual handling planning techniques including written manual handling plans. Experience of innovation and supporting service development projects. Qualifications
BSc (Hons) or equivalent Physiotherapist qualification. Registered Physiotherapist with Health and Care Professions Council Registration. Evidence of ongoing education / professional development.
