Pulmonary Rehabilitation is a twice-weekly exercise and education programme for 6-weeks that is delivered in community sites across Hampshire, covering from Portsmouth to Basingstoke. The programme is designed for people with COPD, Bronchiectasis and Interstitial Lung Diseases (ILDs) who experience symptoms of breathlessness to better understand their condition/s and symptoms to support effective self-management. The purpose of this service provision is to promote and embed Pulmonary Rehabilitation as an essential component in the management of patients with chronic respiratory disease and integrate with other health care services. The service forms part of our Community Respiratory Service, with exposure to multi-disciplinary team members working with Respiratory Virtual Ward, Long Term Respiratory Management and Home Oxygen Service. Main duties of the job

To assist physiotherapists in direct patient care, specifically in the completion of allocated Pulmonary Rehabilitation exercise and education treatment programmes. Work as part of a multi-professional team, in close collaboration with other relevant services/agencies. Undertake routine admin tasks to support the running of the team as requested e.g. taking and making phone calls, emails and those related to our class management. Participate in and maintain own clinical professional development (CPD) by participating in and keeping up to date with the training provided. About us

Qualifications

NVQ II qualification in health or exercise related field Experience

Demonstrate strategies for organising own workload Demonstrate and provide evidence of skill development and provide examples of good practice where key skills have been applied Ability to act on own initiative and take responsibility for own actions Able to use strategies to deal with difficult situations and appropriately seek support and advice when required Shows flexibility and ability to work within a changing environment Good verbal and written skills Understanding of health & safety responsibilities Understanding of Data Protection Experience working with older people within a healthcare setting Experience of working within a multidisciplinary team in either a hospital or community setting Knowledge of SystmOne or other clinical database Understanding of clinical governance requirements Additional Criteria
