To contribute to the Adult Learning Disability Service in delivering effective patient care. This includes carrying out independent clinical work, undertaking specific skilled support work, and administrative and clerical duties. Working with the therapists to support those with more complex needs and manage a caseload of clients who require general support and advice. This includes motivating and supporting them, their carers, and relatives to make lifestyle changes, providing teaching on approved programmes to reduce the risk of complications and ill-health. Client-facing activity includes individual and group sessions involving a range of physical activities. Promote the benefits of good health and give support to improve people's knowledge, skills, and confidence to improve their lifestyles and manage their conditions by using community development principles/values to address and reduce health inequalities in Derbyshire.
