Working with a team of Physiotherapists and Occupational Therapist delivering a Holistic Therapeutic approach to a diverse community population. You will independently complete Physiotherapy Assessments and devise and manage individualised treatment and rehabilitation programmes under the supervision of Registered Physiotherapists. It is important that you have experience of patients with frailty and long-term conditions and should be able to demonstrate an appropriate level of clinical assessment and reasoning for the role. You will be a highly motivated individual with excellent communication, team working and personal skills. You will need to be organised and be able to manage your own caseload and have the ability to demonstrate these in a pressurised environment. Access to a vehicle and possession of a full driving license is essential. There are CPD opportunities around service development as well as structured supervision, peer support groups and appraisals. You will lead in supervision and appraisals of staff and students within the Therapy team. Main duties of the job

The Rehabilitation Practitioner will work as an active member of a community based team and will provide high quality, efficient and effective service to patients. The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction/supervision of a registered practitioner. They will work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies. The Rehabilitation Practitioner will be proficient, knowledgeable and competent. Educated to Foundation Degree level, or equivalent, they will continue to develop in their role through the acquisition of further skills, knowledge and competences, to be determined by the needs of patients, the service and the team.
