Main duties of the job Provide a clinical pharmacy service to an allocated clinical area in accordance with professional standards and local policies. Take patient drug histories. Review medicines to ensure safe and effective use including review of prescriptions for completeness, appropriateness and legality. Communicate recommendations on drug therapy to doctors, nurses and other relevant healthcare professionals as appropriate. Responsible for medicines supply. Ensure the smooth, swift and safe transfer of patient care, liaising with carers, nursing and residential homes where appropriate and providing clear information on discharge medication (whether verbal or written) to patients or their carers. Formulate patient specific pharmaceutical care plans, taking into account results of investigations, response to treatment, and adverse effects. Advise on therapeutic drug level monitoring for specific drugs. Inform and educate patients about their medicines. Provide pharmaceutical information (therapeutic, legal, procedural) to medical, nursing staff, other healthcare professionals and general practitioners where appropriate. Report suspected adverse drug reactions to the Committee on Safety of Medicines using the yellow card reporting scheme.
The ability to speak Welsh is desirable for this post. Welsh and/or English speakers are equally welcome to apply.

Person Specification Qualifications Essential MPharm degree or equivalent. Registered with the General Pharmaceutical Council. Knowledge of Clinical Pharmacy.
Desirable Membership of the Royal Pharmaceutical Society & registration with the Faculty.
Experience Essential Must be able to demonstrate suitable level of experience working in the managed sector and/or community pharmacy.
Aptitude & Abilities Essential Good verbal/written skills. IT literate. Decision making ability. Problem solving skills. Organisational ability.
Desirable Ability to speak Welsh.
