A well-established and patient-focused GP surgery in Farnborough is seeking a dedicated and experienced Practice Nurse to join their dynamic healthcare team. Role Overview:

The Practice Nurse will play a key role in providing a range of clinical services, including chronic disease management, vaccinations, health screenings, and routine nursing procedures. The successful candidate will work closely with the GP team to ensure the highest standard of patient care in a supportive and collaborative environment.

Key Responsibilities:

Provide general nursing care, including but not limited to, wound care, immunizations, and phlebotomy Manage chronic disease clinics (e.g. diabetes, asthma, hypertension) and provide ongoing patient education and support Carry out health assessments and screenings as part of preventative care initiatives Administer vaccinations and other clinical procedures in line with practice protocols Offer advice and support to patients on various health concerns, promoting health education and self-care Maintain accurate patient records in compliance with legal and practice requirements Work closely with the multi-disciplinary team to ensure seamless patient care and follow-up Participate in training and development activities to further enhance professional skills

Essential Requirements:

Registered Nurse with valid NMC registration Experience working as a Practice Nurse or within a similar primary care setting Knowledge of chronic disease management and general nursing procedures Strong communication and interpersonal skills, with the ability to develop positive relationships with patients Ability to work both independently and as part of a collaborative team Passionate about providing high-quality, patient-centred care Up-to-date knowledge of clinical guidelines and best practices

Desirable Skills:

Previous experience in immunization programs and health promotion Ability to run specific clinics such as Diabetes, Asthma, or Travel Medicine Experience using clinical software (EMIS or SystmOne)
