Job Overview We are looking for a Practice Nurse to complement our dynamic and inclusive 3 site practice based in Surrey. This particular role would be based in our Birchwood site in Horley. Days and hours have a level of flexibility to fit with the candidate's needs. Hours 22-37.5 per week. As a training practice for GPs, PAs, and Nurses, we welcome newly qualified nurses or nurses looking to move into primary care and will provide training and support for the right candidate. Pay is according to experience; we offer the NHS pension scheme, free parking on site, and other excellent staff benefits. Main duties of the job To assist in the service and delivery of the care management of the practice population. Work collaboratively within the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. Conducting health assessments, screening, and triage services, and providing appropriate follow-up care. Undertaking a range of clinical procedures, including wound care, immunisations, cervical screening, and phlebotomy. Supporting patients with chronic conditions such as diabetes, asthma, and COPD. Providing health education and promoting preventative care to patients. Ensuring that all clinical activities are conducted in line with relevant regulations, standards, and guidelines. Prescribing medication for patients in accordance with PGDs and own scope of practice. Reviewing patient medications to enhance compliance. Supporting patients in the use of their prescribed medicines or over-the-counter medicines within own scope of practice. Maintaining accurate clinical records. Ensuring read codes/templates are used effectively. Supporting the clinical team with all safeguarding matters, in accordance with local and national policies. Person Specification : What are the minimum requirements all candidates need to meet? Qualifications and Experience: We require high performing team members to join our team with: Minimum 2 years post-registration experience. At least 1 year’s recent primary and community nursing experience. Experience of nurse-led management of chronic conditions. Experience of implementing protocols and clinical guidelines. Experience of quality initiatives such as clinical benchmarking. Audit experience. Clinical supervision training and experience. Knowledge: Knowledge of management of patients with chronic conditions. Knowledge of accountability of own role and other roles in primary care. Knowledge of health promotion strategies. Knowledge of local and national health policy. Knowledge of clinical governance issues in primary care. Knowledge of patient group directions and associated policy. Knowledge of public health issues. Ability to identify determinants on health in the local area. Skills: Clinical leadership skills. Clinical skills: cervical cytology, immunisation and vaccination, ear care, minor surgery. Change-management skills and ability to support patients to change lifestyle. Communication skills, both written and verbal. Ability to communicate difficult messages to patients and families. Negotiation and conflict management skills. Ability to provide teaching and mentorship in a clinical setting. IT skills. Personal Qualities: Professional approach to work. Good telephone manner. Strong team player. Exercises tact and discretion at all times. Demonstrates initiative to handle any unforeseen events during a shift. Demonstrates flexibility towards new working practices and towards working hours. Job Types:
