Modality Partnership Birmingham Division has a part-time vacancy (30 hours per week) seeking a skilled and compassionate, experienced & qualified General Practice Nurse to join our healthcare team. As a Practice Nurse, you will play a vital role in providing high-quality patient care and assisting in various medical procedures working at Smethwick Medical Centre. Salary for this role is £30,171.13 per annum WTE for newly qualified GPNs, £31,715.86-£37,542.82 per annum WTE depending upon PN experience and £42,294.33 per annum WTE upwards for non-prescriber and experienced GPN. Main duties of the job

The role involves the delivery of evidence-based practice for patients presenting with chronic conditions, and the provision of preventative health care to the practice population. The postholder will be responsible for the care delivered to their patients, demonstrating critical thinking and skills in clinical decision-making in the management of patients. The role is not a regular 9 am - 5 pm job; it requires innovation, flexibility, and commitment. The postholder will be required to work resourcefully as part of the team to ensure tasks are completed. About us

Knowledge

Knowledge of management of patients with chronic conditions. Knowledge of accountability of own role and other roles in primary care. Knowledge of health promotion strategies. Knowledge of local and national health policy. Wider health economy awareness. Knowledge of clinical governance issues in primary care. Knowledge of patient group directions and associated policy. Knowledge of public health issues. Ability to identify determinants on health in the local area. Knowledge of public health issues in the area. Skills

Clinical leadership skills. Clinical skills: cervical cytology, immunisation and vaccination, ear care, minor surgery. Change-management skills and ability to support patients to change lifestyle. Communication skills, both written and verbal. Ability to communicate difficult messages to patients and families. Negotiation and conflict management skills. Ability to provide teaching and mentorship in a clinical setting. Experience

Minimum 1 year General Practice Experience. Minimum 2 years post-registration experience. At least 1 year recent community nursing experience. Experience of nurse-led management of chronic conditions. Experience of implementing protocols and clinical guidelines. Experience of quality initiatives such as clinical benchmarking. Clinical supervision training and experience. Personal Qualities
