As part of our practice development, we are excited to offer a new opportunity to join our Practice Nurse team. You will work collaboratively with HCAs, Phlebotomists, Nurse Practitioners, Registrars, Salaried GPs, GP Partners, and our well-established, supportive administration team. This part-time role entails three fixed working days per week. Our practice is situated in a charming market town, featuring a picturesque town park with views of the River Severn, an ideal spot for a relaxing walk or lunch break! Main duties of the job

To undertake clinical management of patients in accordance with practice protocols, policies, patient group directives and evidence-based practice To undertake health needs assessment of patients in the Practice both on an individual and group basis To deliver health education, health screening and health promotion as both opportunistic and planned interventions Accurate and thorough data collection and recording To work on a rota basis to participate in NHS England initiatives regarding extended working hours/7 day working To ensure all patients, visitors and colleagues are treated with respect, dignity always in a friendly and professional way using clear verbal and written communication Continual personal development in knowledge and skill to ensure the needs of the Practice population are met Extended Access at a premium rate of pay About us

We are a team made up of 9 GP Partners (male and female), 6 Salaried GPs (male and female), 2 Nurse Practitioners, 4 Nurses, 5 HCAs and 6 Phlebotomists, working across two rural sites. We are a training practice with several Registrars and Medical Students. We have a large, knowledgeable administration team, and an excellent team spirit throughout. The Team is dedicated to providing an excellent service to our patients whilst promoting healthy living. We are part of a Primary Care Network consisting of 9 practices. We meet regularly and support each other clinically and non-clinically. Job responsibilities

Manage caseload of patients in a clinical system for a variety of conditions to the patient population as required. To practice within own competencies and knowledge appropriate to the role. Management of patients will include the following direct clinical activity: monitoring including blood pressure and ECG, adult, and childhood vaccination programmes (influenza, shingles, pneumovax etc). Participate in the management and recording of medical supplies, stock expiry dates in treatment and storerooms. Input accurate information into the patients electronic health record in a timely and efficient manner, using approved coding to meet Quality and Outcome Framework (QOF) and Practice targets. Initiate and participate in clinical audit as required. Take personal responsibility to ensure infection control policy/aseptic techniques are up to date and adhered to as required, and participate in infection control audits and visits. Actively participate in significant event analysis (SEA). Maintain testing and recording of treatment room equipment, reporting any malfunctions or damage to the line manager. Maintain level of training, skill, and knowledge to always ensure patient care and safety. Ensure Nurse Registration, revalidation and indemnity are always up to date. Opportunistic health promotion of health care, particularly in relation to diet, smoking, alcohol, and exercise, giving preventative health advice. Contribute towards achieving audit targets for the Practice, both clinical and non-clinical. Support the team in dealing with clinical emergencies. Support the clinical team with all safeguarding matters, in accordance with local and national policies and liaising with external agencies. Demonstrate good practice for equal opportunities, culture, and diversity in relations with colleagues and patients. Chaperoning of patients and clinicians as required. Adhere to all relevant policies including infection control and cold chain management. Using protective equipment and clothing provided and in accordance with practice policies. Apply spill control and decontamination policies. Any other appropriate duties as requested by the Management or Partners. Person Specification

Qualifications

Registered Nurse GCSE grade A to C in English and Maths Experience

Experience of working in a primary care environment Experience of working autonomously Experience of working as a practice nurse Experience of infection prevention and control measures Wound care/removal of sutures and staples Requesting pathology tests and processing results Understand the requirement for PGDs and associated policy Ability to work within own scope of practice and understand when to refer to a GP Womens health (cervical cytology, contraception etc.) Understanding of the importance of stock management Broad knowledge of clinical governance Knowledge of health promotion strategies Immunisations (routine, childhood, and travel) ECGs Venepuncture Use of clinical systems (EMIS Web, Docman) Clear, polite telephone manner Personal Qualities
