We look after an older, stable population with opportunities tolook after complex co-morbidities as well as managing acute care. As a Practice Nurse, you would join our team in looking after ourpatients health needs. You would be well supported within the team. We support training and progressionwith a strong learning culture. We host teaching for GP registrars and medicalstudents. Salary offer is based on experience. The offer will also be adjusted commensurate with extra skills the candidate could contribute. Main duties of the job

As our Practice Nurse you would be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients. Working as a senior member of the practice multidisciplinary team,the post holder will ensure nursing services are delivered effectively to theentitled patient population. The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure. The practice will ensure that the post holder hasaccess to appropriate clinical supervision and an appropriate named individualin the organisation to provide general advice and support on a day-to-daybasis. We have a list size of 6550 and a full team including GPs, Nurses, HCA,ACPs, Clinical Pharmacists, First contact physio, Mental Health Practitioner, Management team, Dispensers, Reception, Secretarial and Admin staff. Primary keyresponsibilities The following are the core responsibilities of the PracticeNurse. There may be, on occasion, a requirement to carry out other tasks. Thiswill be dependent upon factors such as workload and staffing levels: a. Toact as the Infection Prevention Control (IPC) lead and to work to the IPCPolicy providing the implementation of audits, training, support and evidenceof compliance to the practice manager b. Toproduce the production of the IPC annual statement in conjunction with thepractice manager c. Todevelop, implement and embed an effective care management programme for thefrail and housebound patients registered at the practice d. Toassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practice e. Todevelop, implement and embed health promotion and wellbeing programmes f. Toimplement and evaluate individual treatment plans for chronic diseasepatients that promote health and wellbeing g. Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients’ health h. Asrequired, to provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSF i. Asrequired, to diagnose and manage acute and chronic conditions j. Toprescribe medication for patients in accordance with PGDs and own scope ofpractice k. Toreview patient medications to enhance compliance l. Toprovide guidance to patients in the use of prescribed and over-the-countermedications regarding side effects and dosages m. Toco-ordinate the provision of travel medicine services n. Torequest pathology services and process pathology results as required o. Tooversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required p. Tomaintain accurate clinical records in conjunction with extant legislation q. Toensure read codes are used effectively by all members of the nursing team r. Toensure the appropriate maintenance and use of chronic disease registers s. Todevelop, implement, embed and participate in well woman clinics t. Todevelop, implement, embed and participate in well man clinics u. Toensure compliance with the practice chaperone policy v. Toassist GPs with minor surgery when required w. Toprioritise health issues and intervene appropriately x. Tosupport the team in dealing with clinical emergencies y. Torecognise, assess and refer patients presenting with mental health needs z. Toimplement vaccination programmes for adults and children aa. Tosupport patients in the use of their prescribed medicines or over-the-countermedicines (within own scope of practice) bb. Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.) cc. Todelegate clinical responsibilities appropriately (ensuring safe practice andthe task is within the scope of practice of the individual) dd. Tosupport the clinical team with all safeguarding matters, in accordance withlocal and national policies ee. Tounderstand practice and local policies for substance abuse and addictivebehaviour, referring patients appropriately ff. Todeliver opportunistic health promotion where appropriate Secondary responsibilities In addition to the primary responsibilities, the Practice Nurse may berequested to: a. Developand review audit protocols and process for the practice, effectivelyutilising the audit cycle b. Develop,review and implement nursing protocols in conjunction with the partners c.Support all members of the nursing team, providingguidance when necessary, conducting appraisals and training needs analyses asrequired d. Participatein local initiatives to enhance service delivery and patient care e. Supportand participate in shared learning within the practice in order to improvepatient care f.Continually review clinical practices, responding tonational policies and initiatives where appropriate g. Participatein the review of significant and near-miss events applying a structuredapproach, i.e., root cause analysis (RCA) h. Coordinatethe nursing team rota, ensuring sufficient staff are available to meetpatient needs i.Lead the planning and implementation of changeswithin the nursing team, enhancing the provision of services to patients j.Support the practice team with the reviewing andimplementation of practice policies and protocols, ensuring conformance toextant legislation Person Specification

Qualifications

Registered Nurse (Nursing and Midwifery Council). Mentor or teaching qualification. Post graduate diploma or degree (Chronic Disease Management). Nurse prescriber. Family planning qualification. Experience

Experience of working in a primary care environment. Experience of chronic disease management. Experience of working autonomously. Experience of infection prevention and control measures. Experience of working as a practice nurse or community nurse. Experience of managing elderly and housebound patients. Experience of quality initiatives, i.e., benchmarking. Other requirements

Flexibility to work outside of core office hours. Occupational Health clearance. Meet the requirements and produce evidence for nurse revalidation. Evidence of continuing professional development (CPD) commensurate with the role of a Practice Nurse. Skills and Personal Qualities

Wound care/removal of sutures and staples. ECGs. Venepuncture. New patient medicals. Requesting pathology tests and processing the results, advising patients accordingly.
