As a Practice Nurse your main duties will include the following: Help patients with a wide range of long-term conditions and provide high standards of clinical care, working alongside our internal nursing team and other community teams. Perform tasks prescribed and delegated by the GP Partners, working unsupervised to Patient Group Directions, agreed protocols, and within defined competencies. Assess patients and implement any treatment plans or reviews at their own discretion. Report to the Lead Practice Nurse (or other senior members of the practice team) with any concerns/queries. About Boughton Health Centre Boughton Health Centre is a large, well-established practice located in the beautiful and historic city of Chester. We are a supportive, high-performing practice situated in a modern purpose-built GP building, with approximately 13,000 patients. You will work alongside a large Practice Team comprising of 1 GP Partner, 9 salaried GPs, 2 Advanced Nurse Practitioners, 3 Practice Nurses (including 1 Lead Practice Nurse), 2 HCAs and 1 Nurse Associate supported by a well-structured Management Team. Job Responsibilities

Main Responsibilities Provide assessment, screening and treatment services relevant to patients and their families, assisting in the provision of general practitioner services. Ensure actions are consistent with clinical governance systems, monitoring and maintaining health, safety, and security within the practice. Participate in the administrative and professional responsibilities of the practice nursing team, assisting in the implementation of policies, standards and guidelines for nursing care. Support patients and colleagues in adopting sound infection control measures. Perform regular checks on all equipment and drugs and ensure records are up to date. Participate in research/audit within the practice as required. Liaise with health authority staff, social services, and voluntary organisations to provide adequate services to patients. Maintain professional development by attending courses and reading, and participate in an annual appraisal process. Maintain electronic patient records, input data at the time of consultation, and update knowledge of annual QOF requirements. Maintain confidentiality of patient information and follow safeguarding principles in accordance with Practice Policy. General Duties: Act as a chaperone when needed. Nursing of minor ailments. Monitoring blood pressure and Hypertension Management. Peak Flow / Spirometry. Performing ECGs. Phlebotomy and INR testing. Requesting pathology tests: urine culture, swabs and blood tests. Wound care. Ulcer care and dressings. Doppler. Removal of Sutures and Clips. Undertake Health Screening, Surveillance and Health Promotion. Provide support to patients in changing behaviours/habits: diet, exercise, alcohol, smoking cessation. Childhood immunisations. Recognise signs, symptoms and categories of abuse, and be aware of safeguarding procedures, statutory local procedures and points of referral. Chronic Disease Management (Including Diabetes, Asthma, COPD): Assist in maintaining GP contract regarding chronic disease patients. Additional training for chronic disease management can be provided. Work within practice guidelines, ensuring safe practice and acknowledging limitations. Experience

Experience of working in accordance with the NMC Code of Professional Conduct. Proven ability to evaluate the safety and effectiveness of clinical practice. Evidence of appropriate continuing professional development activity. Experience of dealing with vulnerable patients. Understanding of equal opportunity and diversity issues. Ability to establish & maintain effective communication pathways within the organisation, with commissioners, other providers, and key external stakeholders. Significant post-registration experience (minimum of 5 years). Worked within general practice. Recent primary and community nursing experience. Experience of using EMIS Web, Docman & Ardens. Knowledge of patient group directives and associated policy. Qualifications

NMC Registered. Evidence of appropriate continuing professional development activity. Skills & Knowledge

Ability to record accurate clinical notes. Knowledge of clinical governance. Knowledge of Infection Prevention Control including Public Health issues in the local area. Communication, both written and verbal. Time management skills and ability to prioritise, plan and problem solve. Ability to use IT confidently and effectively. Ability to work in a fast-changing environment. Ability to assess and manage risk effectively and safely. Able to work under pressure. Able to multi-task and cope with an unpredictable work pattern. Knowledge of LES/DES/QOF/CQC related clinical and administrative work. Immunisations (routine, childhood and travel). Venepuncture. Management of long-term conditions such as Diabetes, Hypertension, Asthma, Spirometry and CHD. Personal Qualities & Attributes

Ability to work flexibly and enthusiastically within a team or on own initiative.
