Cadwgan Surgery are looking to recruit an enthusiastic and highly motivated practice nurse to join our forward-thinking and supportive team. The surgery is located in North Wales, by the coast. The A55 is a 3-minute drive away providing excellent links to Snowdonia and Chester. The ideal candidate will be an experienced practice nurse with chronic disease specialities, but training will be provided as needed. Main duties of the job Treatment room duties such as wound care and injections COPD reviews Asthma reviews Diabetes reviews Cervical screening Child and adult vaccinations Contraception advice and reviews About us We are a large practice of 12,000 patients. The team includes GPs, clinical pharmacists, nurses, health care assistants, receptionists, secretaries, and administration staff. We also have support from other members of the trust who visit the practice weekly such as occupational therapists, audiologists, and midwives. Job responsibilities Job Title:

RGN Job Summary The postholder will be directly accountable to the lead nurse. The postholder will be responsible for assessing, planning, implementing, and evaluating nursing care in the surgery. The postholder will be expected to work collaboratively with the Practice nursing team, General Practitioners, and other agencies to provide clinically effective care. Key Responsibilities General Management Delivery of care, in order to meet the patient's needs and taking into account patient autonomy. Participate in the PDR process annually. Participate in annual audits. Comply with practice policies, procedures, and guidelines relevant to the area of work. Participate in regular team meetings. Caseload Management Assist the surgery to deliver holistic care to patients in the practice setting. Provide skilled nursing care which involves manual dexterity and coordination including wound care, immunisations, ear care, cervical cytology, patient screening. Participate in the delivery of care involving complex needs and supporting carers and relatives in distressing circumstances. Enhance positive health and prevent or diminish ill health: Through active and opportunistic health promotion, infection control measures, child protection, and family planning. Ensure accurate record keeping in line with practice policy and NMC Guidelines. Adhere to practice policies and guidelines on body fluid, sharps injury, clinical waste, and manual handling. Establish priorities in daily workload to organise patient care and use time effectively. Leadership Support and supervise junior staff. Participate in the development of a shared vision for practice nursing. Promote reflective practices within the team. Training and Education Identify training needs at annual PDR linked with competency framework. Access mandatory training as per training schedule. Strive to complete all competencies. Ensure continued personal professional development and development of portfolio. Resources Assist with maintenance stock control of nursing supplies. Ensure efficient use of resources. Person Specification Knowledge Essential Evidence of ongoing professional training. Knowledge of Professional code of conduct. Broad knowledge of relevant community nursing skills and competencies. Awareness of Practice policies. Desirable Knowledge of General Practice policies and Quality and Outcomes Framework. Awareness of Clinical Governance and Health and Safety. Qualifications Essential Registered nurse. Knowledge of primary care. Desirable Previous experience in primary care. Skills Essential Clinical competency. Effective interpersonal and communication skills. Basic IT skills. Critical analysis skills. Ability to maintain high standards of care. Positive attitude to change. Team worker. Self-motivator.
