Key Responsibilities:
Deliver comprehensive nursing care, including wound care, immunisations, long-term condition management, and health screenings. Conduct patient assessments, develop personalised care plans, and ensure patient-centred care delivery. Collaborate effectively with GPs, healthcare assistants, and other members of the multidisciplinary team to provide continuity of care. Provide health education and promotion to empower patients in managing their health and well-being. Maintain accurate, up-to-date patient records in compliance with relevant guidelines and standards.
