Neasham Road Surgery are looking for an experienced Practice Nurse to work alongside our supportive team of 4 Practice Nurses, 3 HCA's and Nurse Associate. We can provide a friendly and supportive working environment with an allocation of paid study leave and support with any ongoing or further training. We are a large practice providing healthcare to 12,500 registered patients. We would require you to work Tuesday, Thursday and Friday due to room capacity but are flexible on hours worked. We can offer a great hourly rate depending on experience and qualifications of chronic disease conditions. Main duties of the job

We would like applicants to have at least two qualifications in chronic disease areas such as respiratory, diabetes and/or family planning, to compliment our existing team. Childhood immunisations is also required along with routine nursing duties. We would expect the successful candidate to take part in clinical meetings and provide a high quality service to our patients. To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. Job Responsibilities: Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs Chronic disease management in at least two disease areas (respiratory, diabetes, family planning, CHD, hypertension) Urinalysis ECGs Venepuncture Blood pressure measurements Routine immunisations Requesting basic pathology tests, for example urine culture, swabs using ICE Following agreed clinical protocols with referral to senior nurses or GPs as appropriate Infection Control Other tasks including: Chaperoning and assisting patients who are being examined by another clinician. Undertake housekeeping duties including, general tidiness and cleanliness of nurses and treatment rooms Participation in administrative systems in the Practice A duty to advise senior nurses of potential problems or errors within the range of assigned tasks Maintain accurate records Attend and participate in any Practice meetings when required. Any other delegated duties appropriate to the post. Special Requirements of the Post: An understanding, acceptance and adherence to the need for strict confidentiality. Ability to use own judgment, resourcefulness and common sense. A commitment to maintain a high professional standard of nursing care and keep up to date with all aspects of nursing care relevant to the post. A commitment to ensure all Health and Safety requirements and Infection Control measures are met and to report any problems to the Practice Manager. A commitment to the effective use of Practice and NHS resources. An awareness of own limitations and experience. To work only in accordance with the UKCC Code of Conduct and within the Scope of Professional Practice. To have a written professional development plan and to maintain an up to date portfolio which meets the requirements of registration with the UKCC. Qualifications

GCSE grade A to C in English and Maths Registered General Nurse Qualification in at least two chronic disease areas Experience of working in practice or primary care Experience of other chronic disease areas Experience of SystmOne clinical system Experience

Experience of working in GP practice and/or primary care Willingness to undergo further training, if needed Depending on experience, hourly rate up to £21.36.
