Salary Please note starting salary is £35k pro rata per annum and is dependent upon experience. Job Summary We are looking to recruit a registered Nurse with experience in delivering the management of diabetes. Selsey Medical Practice is part of the Chichester Association of Medical Practice Primary Care Network. We are committed to improving the care provided to patients, reducing health inequalities, and raising the quality and standards of GP practice. This role will be part of the developing model of care which addresses our ambition to deliver person-centred coordinated care and support the implementation of the NHS long-term plan. We would love to hear from you if: You are committed to outstanding patient care. You are an experienced Nurse who is trained to deliver diabetes reviews and ongoing management. Main Duties of the Job Practice nurses are part of our multidisciplinary team delivering chronic disease management clinics. The successful candidate will: Provide compassionate, safe, and effective care and support to patients. Prioritise and manage their own workload, recognising where elements of care can safely be delegated to other colleagues, carers, and family members. Provide and receive sensitive information concerning patients' medical conditions. Overview Selsey Medical Practice is part of the Chichester Alliance of Medical Practices (ChAMP) Primary Care Network, which consists of 8 GP surgeries covering Chichester, Selsey, Wittering, Tangmere, and Southbourne, providing care to over 92,000 patients. We have 12,500 patients registered at our practice, supported by 11 GPs, a team of nurses, and other healthcare professionals. We are a training practice, with 2 GP trainers, always striving to be patient-focused and prioritising keeping people living in their homes, living well, and having choices.
