We are looking for an enthusiastic, friendly, and hard-working General Practice Nurse to join our clinical team. You will work alongside our GPs, Nurses, Healthcare Assistant, and administrative staff to deliver the highest quality of care to our practice population. Whilst experience working in a General Practice setting is desirable, the most important quality we look for is a willingness to get involved in our team. Main duties of the job

Your main duties will be general nursing duties such as (not exhaustive): Long Term Condition Reviews e.g. Diabetes, COPD and Asthma Cervical Screening Immunisations 24 hour Blood Pressure Monitoring Health Screening ECGs Ear Irrigation Experience or a willingness to train in Diabetes and Respiratory is essential. In addition to this, you will be expected to assist with areas of leadership, which may be through infection prevention and control (IPC) compliance, and developing protocols to maximize QOF prevalence. We are a friendly and busy General Practice with approximately 11,750 patients. We are a well-established and dynamic team comprising of 3 GP and one non-GP Partner, Salaried GPs, ANPs, ACPs, Pharmacy Technician, Physiotherapist, Practice Nurses, and Healthcare Assistant who are well supported by an experienced administrative team. We are part of Folkestone, Hythe & Rural Primary Care Network (PCN) encompassing 7 local practices providing additional services to meet our patients' needs in a holistic manner. Job responsibilities

Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being. Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care. Deliver opportunistic health promotion using opportunities such as new-patient medicals. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions. Assess and care for patients presenting with uncomplicated wounds including Doppler Studies and wound assessment. Support and advise women requesting information relating to family planning needs. Support and manage health needs of women presenting for cervical cytology consultations. Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health. Implement and participate in vaccination and immunisation programmes for both adults and children. Advise, support and, where appropriate, administer vaccinations for patients travelling abroad. Promote and deliver evidence-based care for patients presenting with aural conditions. Person Specification

Qualifications

NMC Registered Additional qualifications in relevant areas (e.g., immunisation, diabetes management, asthma care) Experience

Experience working as a Nurse in a Healthcare setting Proficiency in basic nursing Understanding of Primary Care Service Knowledge of Patient-Centered care Knowledge of confidentiality regulations Excellent verbal and written communication skills Ability to communicate with patients, families, and the healthcare team effectively Ability to manage time effectively, prioritise tasks, and manage a busy workload Ability to work as part of a multidisciplinary team and support colleagues Ability to assess, plan, implement, and evaluate patient care independently Ability to remain calm under pressure Self-motivated, reliable, and flexible Basic computer literacy (e.g., Microsoft Office, clinical systems) Eligibility to work in the UK Commitment to maintaining professional development Experience working in Primary Care/GP Practice Experience with a wide range of treatments (e.g., travel vaccinations) Awareness of local healthcare services and resources, as well as community health needs Experience working with diverse populations and delivering education or training Experience in leading or managing a team of healthcare professionals Experience working with EMIS WEB Clinical System Disclosure and Barring Service Check
