Practice Nurse - GP Practice - Full Time

The Practice Nurse will be responsible for a number of clinical areas within the Practice, including infection prevention and control, health promotion, chronic disease management, well women and well man clinics, supervising and assessing students and junior staff members in their CPD, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedures and QOF. To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. Victoria Medical Centre (VMC) must ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis. Main duties of the job

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks, depending upon factors such as workload and staffing levels: Key Responsibilities:

Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients' health. Provide chronic disease clinics and multi-morbidity clinics, delivering patient care as needed and referring patients to secondary/specialist care as required. Administer childhood immunisations in line with current PGDs. Undertake a programme of cervical cytology screening. Provide travel vaccinations and administrative services. Provide hosiery measurements as required. Administer medication for patients in accordance with PGDs/PSDs and within own scope of practice. Undertake opportunistic screening including BP monitoring, urinalysis, weight monitoring, phlebotomy, ECGs, Doppler, and spirometry. Offer suture removal, wound care, and management of patients with leg ulcers. Undertake ear irrigation and provide ear care advice. About us

Clinical knowledge and skills

Wound care/removal of sutures and staples ECGs Venepuncture New patient medicals Requesting pathology tests and processing the results, advising patients accordingly Understanding the importance of evidence-based practice Understanding the requirement for PGDs and associated policy Ability to record accurate clinical notes Ability to work within own scope of practice and understanding when to refer to GPs Chaperone procedure Immunisations (routine, childhood, and travel) Broad knowledge of clinical governance Knowledge of public health issues in the local area Awareness of issues within the wider health arena Knowledge of health promotion strategies Experience

Registered Nurse (Nursing and Midwifery Council) Good standard of general education Mentor or teaching qualification Postgraduate diploma or degree (Chronic Disease Management) Skills
