Sadly due to one of our Practice Nurses leaving due to re-location to elsewhere in the country, an opportunity has arisen for a proactive and enthusiastic practice nurse to join our highly effective and friendly practice team at Horncastle Medical Group. This is ideally for a full time position, however a part time position of a minimum of 20 hrs will be considered.
The role of the Practice Nurse is to provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs/ANPs, and other members of the practice and primary care team.
The ideal candidate will be registered with the NMC and have experience of working in GP practice setting.

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs, and other members of the practice and primary care team. Assess, plan, develop, implement and evaluate treatment plans that promote health and well-being, and for patients with long term conditions. Proactively identify, help in diagnosing and manage treatment plans for patients at risk of developing a long term condition. Work with patients in order to support adherence to prescribing treatments. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients to adopt health promotion strategies that promote patients to live healthy, and apply principles of self-care. Support and manage health needs of women presenting for family planning and cervical cytology consultations. Implement and participate in vaccination and immunisation programmes for both adults and children. Advice, support and administer vaccinations where appropriate for patients travelling abroad.
