Sadly due to one of our Practice Nurses leaving due to re-location to elsewhere in the country, an opportunity has arisen for a proactive and enthusiastic practice nurse to join our highly effective and friendly practice team at Horncastle Medical Group. This is ideally for a full time position, however a part time position of a minimum of 20 hrs will be considered. The role of the Practice Nurse is to provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs/ANPs, and other members of the practice and primary care team. The ideal candidate will be registered with the NMC and have experience of working in GP practice setting. Main duties of the job

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs, and other members of the practice and primary care team. Assess, plan, develop, implement and evaluate treatment plans that promote health and well-being, and for patients with long term conditions. Proactively identify, help in diagnosing and manage treatment plans for patients at risk of developing a long term condition. Work with patients in order to support adherence to prescribing treatments. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients to adopt health promotion strategies that promote patients to live healthy, and apply principles of self-care. Support and manage health needs of women presenting for family planning and cervical cytology consultations. Implement and participate in vaccination and immunisation programmes for both adults and children. Advice, support and administer vaccinations where appropriate for patients travelling abroad. Meet the needs of patients presenting with opportunistic wound care. Person Specification

Qualifications

Registered (first level) Nurse Current NMC registration Evidence of post registration education Educated at diploma, degree level Diploma in chronic disease management Cervical cytology, family planning, sexual health Qualifications/training Knowledge, skills and attributes

Knowledge of accountability of own role and other roles in a nurse-led service Knowledge of health promotion strategies Knowledge of clinical governance issues in primary care Ability to change care management skills to support patients Excellent verbal communication skills, able to communicate difficult messages to patients and families Good written communication skills, clear concise record keeping IT skills, Systm One clinical system Time management, able to organise workload Knowledge of national policy standards which inform and have implications for Practice, NICE, NSF, QOF Knowledge of local and national health policy Wider health economy awareness Experience

Experienced Practice Nurse or a newly qualified nurse with experience of a student nurse placement in Primary Care (training will be provided to the right candidate) Experience of nurse-led management of long term conditions Experience of developing and implementing protocols and clinical guidelines Portfolio evidence of continuous professional development Ability to work as part of a team as well as an autonomous practitioner Multi-disciplinary and multi-agency working Other
