Job summary Modality Partnership Mid Sussex are seeking a skilled and compassionate, experienced General Practice Nurse to join our healthcare team on a full-time / part-time basis (minimum 25 hours per week). Main duties of the job The role involves the delivery of evidence-based practice for patients presenting with a chronic condition, and the provision of preventative health care to the practice population.
