An exciting vacancy has arisen for a part-time experienced practice nurse to work 3 days per week (Tues/Thurs/Fri), but with flexibility to cover annual leave when required. Experience of working in general practice is essential. Experience of using SystmOne is preferable but full training will be given. The role is varied and covers all aspects of practice nursing. Main duties of the job

The candidate should have core knowledge and skills in the management of long-term conditions i.e. Diabetes, Asthma, Dementia, etc. Childhood Immunisations, Travel Vaccinations, wound management, INR, ABPI, smears, and other enhanced services. The candidate should have the ability to work in a team, have good IT skills, and strive to work for continuous improvement in their working role. We are a very friendly rural practice with a list size of approx. 5300. Great Staughton is located five miles from St Neots with excellent transport links to the A1/A14. Our team comprises one GP Partner, two SGPs, one ANP, two PNs, one HCA, and a very supportive patient services team. We are also a dispensing practice. We were rated first in Cambs and Peterborough for patient experience in the 2023 GP Patient Survey. We consider a work-life balance very important and offer a manageable workload. We use SystmOne. To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice, the NMC code of conduct, and professional boundaries to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, and health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure. The following are the core responsibilities of this role: To develop, implement and embed health promotion and well-being programmes To implement and evaluate individual treatment plans for chronic disease patients To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health To provide routine nursing care to patients as required in accordance with clinical based evidence, e.g., NICE To provide wound care (ulcer/Doppler etc.) to patients To request pathology services as necessary To process pathology results as required To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required e.g., diabetes To maintain accurate clinical records in conjunction with extant legislation To ensure read codes are used effectively To maintain chronic disease registers To chaperone patients where necessary To assist GPs with minor surgery when required ensuring consent received To prioritise health issues and intervene appropriately To support the team in dealing with clinical emergencies To recognise, assess and refer patients presenting with mental health needs To implement vaccination programmes for adults and children To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice) To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.) To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual) To support the clinical team with all safeguarding matters in accordance with local and national policies To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately To deliver opportunistic health promotion where appropriate To recognise and work within own competence and professional code of conduct as regulated by NMC To participate in clinical audit and the implementation of changes to ways of working and/or clinical practice as a result of findings, recommendations and action plans To maintain professional registration and practice in accordance with NMC requirements. Post graduate diploma or degree (Primary Care) Experience

Wound care/removal of sutures and staples ECGs Venepuncture New patient medicals Chaperone procedure Requesting pathology tests and processing the results, advising patients accordingly Diabetes Asthma CHD Immunisations (routine, childhood and travel) Understanding the importance of evidence-based practice Broad knowledge of clinical governance Ability to record accurate clinical notes Ability to work within own scope of practice and understand when to refer to GPs Understanding of infection prevention and control measures Knowledge of health promotion strategies Understanding the requirement for PGDs and associated policy Polite and confident Flexible and co-operative Motivated, forward thinker Problem solver with the ability to process information accurately and effectively, interpreting data as required High levels of integrity and loyalty Sensitive and empathetic in distressing situations Ability to work under pressure/in stressful situations Effectively able to communicate and understand the needs of the patient Commitment to ongoing professional development Punctual and committed to supporting the team effort Knowledge of public health issues in the local area Awareness of issues within the wider health arena Disclosure and Barring Service Check
