We are looking for an enthusiastic, experienced Practice Nurse to join our friendly and supportive GP practice. You will be vital in delivering high-quality care to our patient population of approximately 9000. Duties will including chronic disease management, immunisations and screening services. Main duties of the job

As a Practice Nurse you will deliver high quality patient care and organisational qualities which will include: Management of chronic disease clinics Childhood immunisation programmes and all other aspects of immunisations National screening procedures You will also be expected to work collaboratively with the wider Practice team and Primary Care Network. We pride ourselves on delivering an excellent standard of care for our patients and consistently achieve very high scores in the GP Patient Survey, despite the challenging climate in the NHS. The practice is high QOF scoring and CQC rated Good, with excellent long-term staff retention. We are an active member of the Sale Central PCN. We have 4 GP partners, 5 salaried GPs, 2 Practice Nurses, a HCA and a large administration team. We are a training practice and host ST1, ST2, FY doctors. Many of which have returned as a permanent member of the team. Job responsibilities

Administeradult and child immunisations and vaccinations in accordance with national andlocal programmes. Is competent in anaphylaxis and resuscitation techniques.Ensure safe storage, rotation and disposal of vaccines and drugs and adequatestock levels are maintained. Managementof chronic diseases which includes asthma, hypertension, COPD, CVA-TIA, CHD,and CHD risk assessment and management. Initiates the process of diagnosis withpatients suspected to have chronic disease Maintaindisease registers Initiatesand carries our programmes of Health promotion and screening, cervical andsexual health screening. Experience

Experience of working in a primary care environment as a Practice Nurse Experience of working under pressure Clinical knowledge in the following: ECGs Venepuncture Immunisations (routine, childhood and travel) Requesting pathology tests and processing the results, advising patients accordingly Chronic disease management awareness including: Diabetes, Hypertension, Asthma, COPD, CHD, CKD, Stroke Be willing to undertake home visits Understanding the importance of evidence-based practice Understand the requirement for PGDs and associated policy Ability to record accurate clinical notes Ability to work within own scope of practice and understanding when to refer to GPs Excellent communication skills (written and oral) Effective time management (planning and organising) Ability to work as a team member and autonomously Ability to follow clinical policy and procedure Flexible and co-operative Commitment to ongoing professional development Punctual and committed to supporting the team effort Motivated, forward thinker Problem solver with the ability to process information accurately and effectively, interpreting data as required Ability to work under pressure/in stressful situations Meet the requirements and produce evidence for nurse revalidation Evidence of continuing professional development (CPD) commensurate with the role of a Practice Nurse Broad knowledge of clinical governance Knowledge of public health issues in the local area Awareness of issues within the wider health arena Knowledge of health promotion strategies Experience of undertaking audits Experience with clinical risk management Qualifications

Registered Nurse with Nursing and Midwifery Council Post graduate diploma or degree (Chronic Disease Management) Family planning qualification Competency in using Emis Disclosure and Barring Service Check
