Job Summary: A well-established and patient-centred medical practice in Stockton-on-Tees is seeking a skilled and dedicated Practice Nurse to join their healthcare team. The successful candidate will be key in delivering high-quality nursing care to a varied patient population.
Key Responsibilities:
Deliver a wide range of nursing services, including chronic disease management, wound care, immunisations, and health screenings. Conduct patient assessments, formulate care plans, and ensure patient-centred care is prioritised. Collaborate effectively with GPs, healthcare assistants, and the wider multidisciplinary team to ensure continuity of care. Provide health education and promote wellness, empowering patients to take an active role in managing their health. Maintain accurate and up-to-date patient records in line with current protocols and guidelines. Qualifications and Experience:
Registered Nurse (RGN) with a valid NMC pin. Prior experience in primary care or community healthcare is highly desirable. Strong expertise in managing chronic conditions such as diabetes, asthma, and COPD. Excellent communication skills and a proven ability to work well in a team setting. Proficiency in clinical systems and electronic patient records. Benefits:
