Conveniently located with free onsite parking and excellent motorway links for easy commuting. The Role:
Work collaboratively within a dedicated clinical team, including 6 GP Partners, 1 Salaried GP, 1 Paramedic Practitioner, and 1 Advanced Practitioner. Opportunities to develop your own areas of expertise and interests. Potential for additional hours to support Enhanced Access sessions on weekday evenings and weekends. Main Duties Include: Demonstrating a strong clinical interest in respiratory care and taking on the role of clinical lead for respiratory services. Conducting annual reviews for patients with Chronic Disease Management (CDM). Maintaining accurate and up-to-date registers for chronic disease patients. Monitoring the health of patients with chronic conditions and comorbidities in line with practice protocols. Providing advice and support to patients to promote effective self-management. Collaborating with the QOF Recall Team to optimise QOF performance. Taking the lead on Infection Control. Actively participating in the delivery of general practice nursing services. Upholding high standards of safety and cleanliness within the practice. Working alongside The Diabetes lead for management of Diabetic Patients. Providing an adult and child immunisation service. Monitoring and auditing various aspects of the nursing service as required. Contributing to nurse triage and clinical meetings as required. Job Type:
