Job Overview Modality Partnership Nottingham Division has a part-time vacancy (35 hours per week) seeking a skilled and compassionate, experienced & qualified General Practice Nurse to join our healthcare team. As a Practice Nurse, you will play a vital role in providing high-quality patient care and assisting in various medical procedures working at Wellspring Surgery. Main duties of the job The role is both the delivery of evidence-based practice for patients presenting with a chronic condition, and the provision of preventative health care to the practice population. The role is not a regular 9am-5pm job; the job requires innovation, flexibility, and commitment and the postholder will be required to work resourcefully as part of the team to ensure tasks are completed. To plan and deliver high quality integrated nursing care to the practice population. To promote health and wellbeing within the practice population. To work with clinical and non-clinical practice staff, other members of the wider healthcare team and NHSE/ICG in contributing to: The achievement of QOF. Delivering objectives set out in the NHS Action Plan. Audits. Meeting the standards as set out by the Care Quality Commission (CQC). To promote and maintain high standards of nursing care in accordance with the NMC Code of Professional Conduct and Guidelines for Professional Practice. To provide travel immunisations, cervical cytology, ear syringing, wound management. To plan and run health promotion and long-term condition related clinics. To directly communicate with patients on matters relating to their ongoing and highly complex health needs. To ensure that patients have detailed assessments made of their healthcare needs. Facilitate patient referrals to appropriate clinical services. To liaise with other disciplines and outside agencies with a view to providing holistic healthcare. To participate in the delivery and evaluation of healthcare services at the Practice. To be responsible for the ordering, safe systems, and processes, storing and administering of all medicines/drugs according to NMC standards. To undertake and participate in clinical audit projects as necessary to own work. Respond to medical emergencies. Ensure that the principles of Infection Control are always adhered to. To have a good awareness of issues concerning child protection & vulnerable adults. To keep abreast of advances in clinical practice and identify personal training and development needs. Support colleagues in their development by encouragement, informal teaching and coaching. To attend meetings as directed by the Practice Manager and Partners. To attend mandatory training identified through the appraisal system. Contribute and work within the broad framework of the clinical governance agenda. Work within evidence-based nursing protocols. Personally, and accurately record all healthcare activity within the patients clinical records. Ensure that communication is effective between all stakeholders. To work in a flexible manner to meet the needs of the service and as directed by senior staff and GPs. What are the minimum requirements all candidates need to meet? Qualifications and Experience: Minimum 1 years General Practice Experience. Minimum 2 years post-registration experience. At least 1 years recent community nursing experience. Experience of nurse-led management of chronic conditions. Experience of implementing protocols and clinical guidelines. Experience of quality initiatives such as clinical benchmarking. Audit experience. Clinical supervision training and experience. Knowledge: Knowledge of management of patients with chronic conditions. Knowledge of accountability of own role and other roles in primary care. Knowledge of health promotion strategies. Knowledge of local and national health policy. Wider health economy awareness. Knowledge of clinical governance issues in primary care. Knowledge of patient group directions and associated policy. Knowledge of public health issues. Ability to identify determinants on health in the local area. Skills: Clinical leadership skills. Clinical skills – cervical cytology, immunisation and vaccination, ear care, minor surgery. Change-management skills and ability to support patients to change lifestyle. Communication skills, both written and verbal. Ability to communicate difficult messages to patients and families. Negotiation and conflict management skills.
