Practice Nurse Reporting to:
Senior Nurse PURPOSE OF THE ROLE To work as a Practice Nurse within the organisation taking responsibility for assessing the nursing needs of patients attending surgery with a broad range of long term illnesses. All duties and responsibilities to be carried out in accordance with protocols developed by the Nursing Team and in agreement with the Nurse Clinical lead. To provide a high quality, evidence-based easily accessible service for patients. To be able to time manage during clinics to ensure a seamless service that is delivered on time, as far as reasonably practical, to the patients. To work with the Senior Nurses in liaison with the site Team Leaders on a day-to-day basis and particularly with regard to arrangements for patient recall and review, holidays and rotas, and as a member of the surgery team. Main Duties and Responsibilities
To provide treatment room nursing care, including wound management, venepuncture, ECG, routine injection and urinalysis. To assess and treat minor illness and injury in accordance with protocols, discharging or referring patients as appropriate. To provide a triage service for patients, including by telephone, offering advice if appropriate in accordance with protocols. To organise medication and wound dressings with a GP / Non-Medical Prescriber as part of a plan of care for an individual patient as needed. To assess those patients wishing to travel, offering basic vaccination, malaria prophylaxis and advice about where to seek more specialist advice. To provide contraceptive, sexual health, coil check and menopause advice and administer depo-provera injections according to protocol. To provide a cervical screening service in accordance with the National Policy and local NHS recall system. To carry out routine swabs as part of a request by GP / ACP as required. To provide childhood and adult immunisations in accordance with the British Vaccination Schedule. To provide screening examinations e.g. new patient health check, elderly screening and lifestyle advice as requested by GP colleagues or as part of a recall system. To visit patients in their homes to undertake work on behalf of the Practice as requested by the GP / Nursing Services Manager / Lead Practice Nurse. To assist in the provision of care to those with long term conditions such as Coronary heart disease, hypertension, Diabetes, COPD, Asthma. To be responsible for the maintenance of accurate records, reflecting consultations and communications with patients. To ensure nursing care is evidence based and evaluated appropriately. To act as a representative for Belvoir Health Group at PCN and ICB level, as delegated by Nursing Services Manager. To take responsibility for the teaching and supervising of the Health Care Assistant, phlebotomists and Nursing students as required. To attend designated meetings and relevant training courses keeping professionally up-to-date. To adhere to the in-house complaints procedure. To ensure that the clinical room is kept tidy, and instruments are sterilised as required. Any other duties within the range and scope of this Job Specification as deemed necessary and appropriate.
