Minor Illness Practitioner
– See patients in ‘same day care’ clinics, e.g., minor injury and illness, abdominal pains, chest pain, tiredness, and headache.
– Perform specialist health checks and reviews.
– Support the delivery of ‘anticipatory care plans’ – a process designed to support patients living with a chronic long-term condition to help plan for an expected change at some time in the future.
– Undertake acute home visits on behalf of GPs, especially for local elderly or immobile populations.
– Lead the team of clinicians that manage the Acute List under the supervision of the Duty Doctor.

Qualifications
Registered Health Care Professional with a recognised Council.
Strong communication skills to effectively interact with patients and healthcare teams.
Proficiency in English for clear documentation and patient education.
IT skills for managing electronic health records and care plans.
Experience in teaching or training to support patient education and health promotion.
Ability to develop and implement individualized care plans tailored to patient needs.
