We are looking for a Mental Health Practitioner to work within Billingham and Norton Primary Care Network (PCN). The Mental Health Practitioner will work closely with the GP practice staff, their TEWV colleagues, and other agencies, especially with the new Additional Roles Reimbursement Scheme ( ARRS ) Workers. The post holder will need to have significant experience working with patients with mental illness and will be responsible for short-term treatment of patients referred to you in a GP setting. Ideally, you will have skills in delivering front-end interventions to aid patients in staying within a primary care setting. Candidates must have 18 months post-qualifying experience, including sign-off of the appropriate preceptorship or competency framework. Main duties of the job

Main duties of the role will include: Work with patients who have severe mental illness and/or complex emotional needs who do not meet the criteria for secondary care services and are too complex for IAPT services to support shared decision-making about self-management; facilitate onward access to treatment services; and provide brief evidence-based psychological interventions, where qualified to do so, where the intervention can be safely offered outside of a multi-disciplinary mental health team. Work closely with other PCN-based roles to address wider patient needs, e.g., PCN clinical pharmacists for medication reviews, and social prescribing link workers for access to community-based support. Operate without the need for formal referral from GPs, including accepting some direct bookings where appropriate. Provide a consultation, advice, triage, and liaison function, supported by the local community mental health provider through robust clinical governance structures to maintain quality and safety, including supervision where appropriate. Supervision, training, and personal development are key aspects of the post, along with regular supervision/appraisal, which we see as an important aspect of the role. About us

TEWV offers a range of support for you such as: We are the Mental Health & Learning Disability NHS Trust for County Durham and Darlington, Teesside, North Yorkshire, York, and Selby. From education and prevention to crisis and specialist care, our talented and compassionate teams work in partnership with our patients, communities, and partners to help the people of our region feel safe, understood, believed in, and cared for. We nurture the recovery journey of anyone in need of our help. In Our Trust, everyone has a say in how they are supported and treated because we listen to every person in our care until they feel understood. Our patients, their families, and carers work together with us towards better mental health. We're committed to new thinking that improves the wellbeing of our region. We connect with our communities and partners to get mental health care right in areas that really need it. We won't rest until everyone in our region has the mental health care they need to lead their best possible life. Job responsibilities

As part of the Community Transformation, this role is an essential aspect of patient care and journey. You will work closely as a central point with many other agencies/TEWV services. Please refer to the attached job description and person specification for further detailed information to ensure that you meet the role criteria before applying. Person Specification
