Bringing general practicestogether to work at scale has been a policy priority for some years for a rangeof reasons, including improving the ability of practices to recruit and retainstaff; to manage financial and estates pressures; to provide a wider range ofservices to patients and to more easily integrate with the wider health andcare system. West Leeds Primary Care Network (WLPCNLtd) is a large PCN with a patient list of circa 70,000 patients around thePudsey / Bramley area. We have a significant workforce in place which bringswonderful opportunities for in-house multi-disciplinary team working. We have awell developed multidisciplinary team including a Pharmacy Team, Occupational Therapists, Heart failure nurse specialist, Social Prescribers,Paramedics, Dietitians, a range of nursing / HCA staff and more which meansthat the majority of any patient's needs can now be provided on their own doorstep.
