To support our practices and ensure patients receive the bestquality care West Leeds PCN is seeking an experienced Diabetic Nurse Specialistto work alongside our Senior Clinical Pharmacist and HF Nurse, the FrailtyTeam, and clinicians within practices to develop and deliver a comprehensiveDiabetic service for the patients within the PCN. Tobe responsible for the management of diabetes including insulin initiation,health education, reviewing bloods, medication monitoring and doseadjustment. Working as part of the PCNmultidisciplinary team, delivering care within their scope of practice to theentitled patient population. The role will include supporting the managementteam in the reviewing of clinical policy and procedure. The post holder will deliver clinic and home-based treatment plansincluding education and lifestyle advice, monitoring and management of symptomsand medications. The service seeks to provide access to diagnostics and earlytreatment intervention. The post is integral to the multidisciplinary team,working closely with our partners in secondary care to provide seamlesstransition for patients to primary care underpinned by a solid clinicalgovernance structure. The post holderwill build relationships with wider external organisations to promote joined uppatient care and services. Main duties of the job

KeyResponsibilities Responsiblefor the assessment, planning and implementation of clinicalmanagementplans. Deliverdirect care to patients including providing personalised planned interventionsto support people to remain safe, well and as independent as possible in theirown homes. Initiateinsulin and proactively monitor the care of all diabetic patients in line withlocal and national policy. Provideeducation and training to health care professionals across the pathway anddeveloping the focal point for responsive, high quality, evidence basedclinical advice in the out of hospital system, closely integrated withsecondary care specialities. Worksin partnership with other groups in the development of innovative clinicalpractice. Assistsin managing a caseload, in line with PCN policies, guidelines and nationalstandards. Primary Care Networks (PCNs) form akey building block of the NHS long-term plan. Bringing general practicestogether to work at scale has been a policy priority for some years for a rangeof reasons, including improving the ability of practices to recruit and retainstaff; to manage financial and estates pressures; to provide a wider range ofservices to patients and to more easily integrate with the wider health andcare system. West Leeds Primary Care Network (WLPCNLtd) is a large PCN with a patient list of circa 70,000 patients around thePudsey / Bramley area. We have a significant workforce in place which bringswonderful opportunities for in-house multi-disciplinary team working. We have awell developed multidisciplinary team including a Pharmacy Team, Occupational Therapists, Heart failure nurse specialist, Social Prescribers,Paramedics, Dietitians, a range of nursing / HCA staff and more which meansthat the majority of any patient's needs can now be provided on their own doorstep. We have a strong ethos of trainingand development and host 3rd & 4th year medicalstudents and GP registrars. As a well established and mature PCN we have astrong culture of governance and clinical supervision. We have a GP educationallead, A CVD lead, a Lead Practice Nurse and a Lead Practice manager, who, alongwith the Clinical Director form the PCN executive. Job responsibilities

Service Description: WestLeeds Primary Care Network (WLPCN) provides enhanced clinical services for sixGP surgeries across the Pudsey and Bramley area,We have a higher than average prevalence ofpatients with Type II diabetes and seek to employ a Specialist Diabetic Nurse tosupport practices in delivering excellent diabetes care and developing a multimorbidity approach to Long term condition management West Leeds PCN currently benefits from the following clinicalteams, directly employed by ourselves: Pharmacistsand Pharmacy Technicians HealthCare Assistants OccupationalTherapists LearningDisability / Severe Mental Illness Care Co-ordinator Highlyskilled prescribing Frailty / Home visiting team (Advanced Clinical Practitioners andAssociateClinical Practitioners) Heart FailureNurse Thesuccessful recruit will be expected to work across all practices in thePCN. Qualifications

Registered Nurse with active NMC registration. Independent prescriber with experience in GLP1 & insulin initiation A car driver and access to own car. Disclosure and Barring Service Check
