The main purpose of this role is to provide proactive clinical support to vulnerable patients and those with complex physical or mental health needs who are at high risk of future hospital admission. The aim is to improve their quality of life and healthcare experience and therefore reduce the risk of emergency hospital admission. A community-based role, the nurse will visit patients in their own homes and be responsible for the patients' holistic care in conjunction with the patients' own GP Surgery.
