What You Will Do: Conduct comprehensive mental health assessments for adults in the community, developing individualised care plans. Deliver high-quality, person-centred care, utilising a range of psychological theories and frameworks. Collaborate with multidisciplinary teams to ensure integrated care is delivered to clients. Provide support and guidance to patients and their families, helping them navigate mental health challenges. Maintain accurate, up-to-date clinical records and report on patient progress. Facilitate group therapy sessions, workshops and educational forums within the community.
