Benefits of the role: Generic caseload Competitive rate Supportive team Responsibilities: Assessments and Support Planning Conduct comprehensive assessments of individuals’ needs, considering physical, emotional, and social factors. Develop personalized support plans that empower clients to achieve their goals. Collaborate with health professionals, care providers, and community resources to coordinate services. Safeguarding and Risk Management Identify and respond to safeguarding concerns promptly. Assess risks related to vulnerable adults and implement appropriate interventions. Work within legal frameworks to ensure the safety and well-being of service users. Direct Work with Clients Provide emotional support, counseling, and advocacy. Assist clients in accessing relevant services, benefits, and housing. Facilitate group sessions and workshops to enhance social skills and resilience. Collaboration and Multi-Agency Working Liaise with healthcare professionals, housing officers, police, and other agencies. Attend case conferences and multidisciplinary meetings. Advocate for clients’ rights and needs within the wider system. What we are looking for: Social Work England registration Eligible to work in the UK Hold a full UK licence Post-qualified experience Degree level or equivalent in Social Work Please note:
