Role Overview: As a Social Worker in this pivotal position, you will work closely with Children's Services, Education, and Health to provide essential support for young people with complex needs. You will conduct Care Act 2014-compliant assessments, develop person-centred support plans, and advocate for the rights and welfare of young people and their families. With a focus on assessment and support planning for high-cost placements and care packages, your role will ensure that young people transitioning into adult services receive the appropriate support.
Key Responsibilities:
- Carry out eligibility assessments under the Care Act 2014.
- Develop strengths-based, person-centred outcomes within support plans.
- Collaborate with NHS partners to secure Continuing Healthcare (CHC) funding for eligible individuals.
- Monitor and review support plans to ensure they meet changing needs.
- Safeguard the welfare of young people by adhering to statutory frameworks and legislation.
- Manage a varied caseload, making sound professional judgments.
- Work closely with multidisciplinary teams to provide holistic care.
