Qualified Social Worker, Care Coordinator, Kirklees Council, Adult Community Mental Health Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team. The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager. Responsibilities include: Assessments Responsible for a caseload of service users with complex mental health needs Develop and implement care plans and review and assess these Experience of working with people with mental health needs, care planning, and risk assessments is required Knowledge of statutory responsibilities, i.e., Care Act, safeguarding vulnerable adults and children, etc. Liaise with other professionals 36 hours per week (flexible by negotiation) 2 Month Temporary Contract Start date subject to DBS and reference clearance ****************************************************************************************** Successful candidates will benefit from: A Kindle Fire upon commencement of contract (see terms) £20 Amazon voucher if registration is completed within 2 weeks (see terms) Competitive pay rates Weekly payroll Free access to a comprehensive online training portal worth £500 a year Your own dedicated consultant with extensive social work knowledge Receive up to £250 for each referred worker (see terms) ****************************************************************************************** The successful candidate will need to hold a DipSW or equivalent, must be registered with the HCPC, have a current DBS, and be a car driver.
