The team provides short term interventions to maximise independence and responds to urgent situations to enable people to continue living in their own home for as long as possible. The team is made up of a team of dedicated health and social care staff, working together to provide holistic assessment and support plans to ensure the best possible outcomes. You will carry out assessments under the Care Act 2014 to determine eligibility need Maximise rehabilitation services such as reablement & care where appropriate Provide assessment and appropriate interventions to aid patients in achieving optimum recovery/rehabilitation and quality of life. Work with patients and families addressing the impact of illness and treatment To be considered for the Social Worker role, ideally you will have:
