Sutton Health and Care (SHC) Reablement Unit aims to support discharge and minimise length of stay by providing intensive multi-disciplinary rehabilitation to patients in the new Reablement Unit and their own homes. Post holders will be working as part of an integrated multidisciplinary team including nurses, occupational therapists, physiotherapists, social workers, rehabilitation assistants, Reablement assistants, and administrators as well as the voluntary sector. The post holder will be based in the new Reablement Unit but may also have the opportunity to work in other areas, which include the rapid response hospital hub (the Emergency Department, Acute Medical Unit, and Acute Frailty wards of St. Helier Hospital), rapid response community, and intermediate care (bedded and home-based care). Main Duties of the Job
To assist in the identification of health and social care needs, and participate in the delivery and evaluation of rehabilitation under the indirect supervision of a registered health or social care professional both in the Reablement Unit and in patients' own homes. To work as part of a multidisciplinary team within the (SHC) Reablement Unit and to assist in the development of good working relationships with statutory, voluntary, and independent organisations. To maintain clinical skills and competencies by working with both the acute and community rehabilitation functions of the service. To provide supervision and assessment for junior staff and to contribute to the development of this role within the Trust and SHC Reablement Unit. About Us

Previous experience working with Therapies within health or social care. Demonstrate history of working as part of a MDT. Experience of inpatient or community patient rehabilitation. Skills and Abilities
Evidence of good organizational skills and ability to manage workload autonomously. Basic clinical reasoning and problem-solving skills. An understanding of common medical terms and conditions. Previous EMIS experience. Personal Qualities
