This is mainly an administrative role to monitor and administer the practice recall systems, actively inviting patients for their long-term condition reviews and other appointments as required. The successful candidate will need to have excellent attention to detail, good IT skills, work well both in a team and under their own initiative, and have a positive can-do attitude. About Us

IG Medical is situated in West Yorkshire, with 2 sites: Ilkley Moor Medical Practice set in the picturesque town of Ilkley and Grassington Medical Centre in the heart of the Yorkshire Dales. We are a training practice and a caring, forward-thinking, committed team, whose aim is to improve the health of our community by delivering high-quality patient care while maintaining a fulfilling working environment for all. Our patient list size is currently around 18,000. Our team consists of: 5 GP partners 2 Advanced Nurse Practitioners 1 Advanced Clinical Practitioner A nursing team which consists of both Practice Nurses and HCAs Management team Data Admin Team Reception Admin Team MSK Admin team Job Responsibilities

The following are the core responsibilities of the

Care Coordinator for Recalls and Administration . There may be occasional requirements to carry out other tasks; this will depend upon factors such as workload and staffing levels: Full management of the long-term conditions reporting Maintain a good working knowledge of current QOF and recall processes and requirements Administer the patient invitation process for long-term condition reviews and other appointments as required Monitor patient recall and recall systems Ensure accurate report output Deal with queries via email, telephone, and SystmOne tasks relating to Long Term Conditions patients and recalls Ensure patient records are updated accordingly Liaise with the practice clinical team Liaise with patients, relatives, hospitals, and other agencies Person Specification

Experience

Experience of working with the general public Experience of administrative duties Experience of working in a healthcare setting Qualifications

Educated to GCSE level or equivalent GCSE Mathematics & English (C or above) NVQ Level 2 in Health and Social Care Disclosure and Barring Service Check
