Barrack Lane Medical Centre has an opportunity for a care navigator to join our friendly, supportive and professional team for approximately 30 hours per week over three days. Although you will be employed by the Barrack Lane & Ivry Street PCN, you will be based at Barrack Lane. The care navigator is a very important role within the surgery as they are the first person a patient speaks to or sees when they contact the surgery. General practice is a challenging environment that requires a calm and caring approach with the ability to work under pressure. The ideal candidate will be a highly motivated team player who has customer service/patient experience and offers excellent communication skills. Sorting all clinical post and prioritising for the GP in terms of actions. Extracting all information from clinical letters that needs coding and adding to notes. Arranging appointments, referrals, tests, and follow-up appointments for patients. Preparing patients prior to going in to see the GP, taking a brief history and basic readings in readiness for the GP appointment. Explaining treatment procedures to patients. Filing and allocating test results, with clinical support. Working with our care co-ordination team. Working to achieve QOF and IIF outcomes. Supporting the practice team to help patients with their chronic disease management reviews. About us

Barrack Lane is a busy practice in the town centre of Ipswich, Suffolk. We have a practice population of over 20,000 patients. The practice is based on two sites: the main surgery in the centre of the town and a branch surgery at Stoke Park. Barrack Lane is part of a PCN (Primary Care Network) with Ivry Street Medical Centre in Ipswich, and the two practices cover a population of 32,000 patients. The PCN allows the practices to share resources and offer an enhanced service to our patients. The PCN employs care co-ordinators and social prescribers to work with patients, especially our vulnerable patients, to help them access the best care. Although you will be employed by the Barrack Lane & Ivry Street PCN, you will be based at Barrack Lane. Person Specification

Qualifications

None Experience

You will need to be focused and conscientious, with excellent attention to detail and be able to work comfortably in a team as well as independently. Previous experience of working in primary care and using SystmOne would be advantageous. Disclosure and Barring Service Check
