Job Summary Are you an experienced recovery practitioner looking for an opportunity to work more independently by expanding and strengthening pathways from inpatient settings to Via’s community support?
The Role With a primary focus on increasing the engagement of people who are referred to Via whilst an inpatient in hospital, you will be responsible for: Delivering a regular schedule of visits to inpatient settings across the borough of Redbridge, visiting both existing participants of Via, and engaging with inpatients who would benefit from engaging with Via at discharge. Using effective motivational interviewing and other key skills, to develop a working alliance with inpatients that encourages them to attend Via at discharge. Supporting seamless continuity of care for existing and new people who use our service coming to Via following an inpatient stay, including support to access relapse prevention medications and groups, and ‘hand holding’ to their first appointment. Holding a caseload of people referred whilst inpatient, to ensure that communication with professionals in Via and other agencies, is coordinated and updated regularly and they are assessed in a flexible way to commence appropriate treatment with Via at discharge. Maintaining a high standard of professional communication with our valued secondary care services, to ensure Via is viewed as a collaborative, caring, and responsive partnership supporting people who are experiencing poor physical or mental health and/or inequalities and barriers to accessing treatment. You will play a key role in maintaining strong partnerships with hospital and secondary care settings across the borough, including hospital drug and alcohol liaison services, inpatient psychiatry wards, and home treatment teams. This new role will provide you with significant scope for growth, innovation and some autonomy, within a supportive and friendly multidisciplinary team of clinical and non-clinical professionals. You will have the opportunity to progress through Via’s pay-scales by completing our Tackling Substance Misuse qualification, as well as access a wide range of other learning and observation opportunities.
The Service Via deliver a range of services supporting the health and wellbeing of our diverse communities in the London Borough of Redbridge - including adult and young people’s drug and alcohol services, sexual health, smoking cessation, and an outreach and rough sleeper team.
Location Our main hub is in Ilford Town Centre, a short walk from Ilford Station on the Elizabeth Line, and served by many bus routes. There is also reduced parking costs available nearby. This role involves regular travel across the borough of Redbridge to secondary care and other settings to engage proactively with people and inpatients. Our main hours of work are Monday-Friday 9am-5pm, with some evening sessions supported on rotation with other colleagues.
