Job Purpose:

To assist in the provision and promotion of full nursing care support to all residents, tailored to individual client needs. Promoting a person-centred approach.

Job Summary:

To undertake a variety of activities in line with residents' individual needs and in line with care plan and requirements of the management, CQC and NMC registration within the Home, ensuring the best possible individual person-centred care for each resident, meeting all nursing and clinical requirements.

Main Duties and Responsibilities/Accountabilities:

To cover such comparable duties as may be required by the Management and Lead Nurse, as well as additional individual assigned responsibilities.

To coordinate full day-to-day nursing and clinical care support to all residents and support the management, as well as the ethos of the company in providing outstanding person-centred care.

To oversee all aspects of care provisions, evaluate practices and procedures with management, and implement change where required to improve the service and benefit all residents.

Ensure that all residents' needs are responded to promptly, in line with individual preferences and the home's policies and procedures, maintaining the safety and welfare of the residents.

To undertake preadmission assessments, where requested by the Management and/or Lead Nurse, and complete all care documentation and risk assessments required to meet clinical needs.

Ongoing review and monitoring of care plans and relevant assessments.

Follow policies, procedures, and working systems as per Management request and needs of the Home.

To provide all aspects of care for the residents' daily living in accordance with agreed ways of working, policies, procedures of the Company, individualised care plans, and residents’ personal preferences.

Maintain and manage residents' care plans and risk assessments, ensuring they remain relevant and up to date.

To take appropriate action to comply with policies and procedures of the Home, ensuring the well-being of all service users, reporting to management.

To manage and support the team with all aspects of required care of the residents, where required.

To ensure multi-disciplinary professionals are liaised with for the welfare of all residents, dependent on needs and changes in well-being are recorded and actioned promptly.

To report all incidents and accidents directly to the Management/Lead Nurse and complete all relevant records.

Report and deal with any observations concerning the comfort and welfare of individual residents promptly, clearly, and accurately, ensuring all relevant information is passed onto the management of the home.

Providing clear and accurate information to external working agencies.

To undertake enquiries in the absence of the Management/Lead Nurse.

To recognise and deal with problems proactively, using personal initiative, whether in nursing or household departments, liaising with the Management team as per policies and procedures.

To ensure high standards of communication with the management team and Lead Nurse.

Working with all heads of departments to promote and support teamwork, thus promoting good working relationships.

To be able to work under pressure due to unpredictability of workload, having a flexible approach to change in duties dependent on the needs of the residents and home.

To assist in the planning and implementation of changes within the service provided by the Home to ensure continued improvements.
