Band 5 Nurse - Hybrid Virtual Ward and Community Care

Join our dynamic clinical team as a Band 5 Nurse in a fast-paced role that combines autonomous clinical practice with collaborative teamwork. You will work closely with the Ward Manager, Deputy Sister/Charge Nurse, Clinical Teams, Lead Nurse, Practice Development Team (PDT), and the Multidisciplinary Team (MDT) to ensure patients receive the highest quality of care across all stages of their care pathway. Through your role, you will not only support and treat patients but also enrich the patient experience, uphold clinical standards, and contribute to professional knowledge sharing.

This position is ideal for a registered nurse with a passion for providing high-quality patient care across multiple settings, who is eager to advance their clinical expertise and leadership capabilities. As part of our team, you'll play a critical role in shaping the future of patient-centered, responsive, and integrated care.

The applicant must have the means to travel around the community, to attend patients in their own home and work for on all trust sites.

Main duties of the job Requirements:

Live NMC Registration

Strong clinical assessment skills with the ability to work autonomously and respond to complex patient needs on the virtual ward and in the community.

Flexibility to work across various settings and provide support remotely, on-site at the hospital, or at patients' residences.

Access to reliable transportation to travel to patient homes and all Trust sites as required.

Commitment to professional development, self-assessment, and continuous improvement in line with Trust and NMC standards.

Deliver high-quality, evidence-based care to patients in a hybrid setting, treating them in their homes, within hospital clinical areas, and remotely using digital monitoring tools.

Conduct thorough clinical assessments, work independently, and recognize signs of patient deterioration in real-time.

Collaborate effectively with the MDT to coordinate patient care, ensuring timely referrals and comprehensive discharge planning.

Act as a clinical role model, demonstrating professional integrity, accountability, and adherence to the highest standards in patient care.

Take charge of the ward as needed, under the guidance of the Ward Manager, and support the ongoing development and management of the ward environment.

Maintain clinical credibility, continually enhancing your skills through the Senior Band 5 competency framework to progress towards becoming a Senior Staff Nurse.

Person Specification Education and Qualifications Essential

NMC Registered Nurse

Experience Desirable

Experience of teaching junior staff

Experience of supporting/mentoring junior staff

Experience of shift management in the absence of senior colleagues

Experience of audit processes

Key Skills/Abilities Essential

Ability to communicate clearly in English in writing, via IT, verbally and using the telephone

Problem solving skills

Peripheral cannulation skills

IV medication administration

Understanding of nursing care delivery

Management of acute patients

Desirable

Understanding of discharge planning

Other Essential

Awareness and respect for colleagues, patients and relatives' cultural, religious and emotional needs and beliefs.
