Overview:

Responsibilities:
Oversee the Care Team and ensure compliance with standards and guidelines
Provide high-quality patient care in accordance with established protocols and standards
Administer medications and treatments as prescribed by healthcare providers
Monitor and assess patient conditions, reporting any changes to the medical team
Collaborate with interdisciplinary teams to create individualized care plans
Educate patients and their families on health management and disease prevention
Maintain accurate and detailed records
Experience:
Required:
At least 2 years experience in nursing home setting
Nice-to-have:
Familiarity with electronic health records systems
