reports to the Director of Care. Coordinates and oversees all aspects of resident-focused delivery of care. Provide direct supervision, leadership, and education to other members of the healthcare team. Acts as a resource and advocate for residents, families, and staff. Promote the values, goals, and philosophy of the Home. Responsibilities: In collaboration with the Clinical Practice Coordinator/designate, provides hands-on support to Registered Staff in each required program area (i.e. Skin Care & Wound Management, Falls Prevention & Management, Continence Care & Bowel Management, Infection Prevention & Control, Palliative Care Philosophy & Pain Management). Conducts weekly MD rounds on designated days and times and transcribes, processes, and assists with completing physician orders. Notifies physician of changes in resident’s health status, as required, and notifies substitute decision maker when there is a change in the resident’s medication/treatment order in conjunction with the unit nurse. Facilitates the admission audits on the day of admission and hands them over to the nurse manager for completion. In collaboration with the program leads, conducts regular audits for all required programs and follows up on areas of non-compliance. In collaboration with the Unit Supervisor, facilitates ongoing monitoring and auditing of annual physicals & heights, residents’ expressed wishes, lab orders and availability of nursing supplies. Conducts unit rounds at the beginning, throughout, and end of shift. Actively participates in the medication reconciliation process and guides staff regarding best practices. Acts as a mentor and coach for registered Staff in clinical areas, communication, personal and professional development. Identify areas of education for capacity building in Registered Staff. Other duties as assigned and or indicated in Job Task Inventory. Qualifications: Registered Nurse with a current Certificate of Registration from the College of Nurses of Ontario, in good standing. Basic CPR. Experience in long-term care setting an asset. Ability to work with and relate to interdisciplinary team members. Well-developed written and oral communication skills. Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one (1) year of being hired. To show evidence of being focused on people’s feelings when interacting and knowing how to turn a task into positive social interaction. To undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk. Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one. As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements.
