We are seeking a Band 5 Nurse to join either York and Shar (South Hambleton and Ryedale) Community Response Teams to deliver high standards of multi-disciplinary interventions to patients within their own place of residence, to prevent avoidable admissions to acute care and to support discharges. Our well-established teams cover the historical city of York, as well as the rolling countryside within North Yorkshire. The team consists of Physiotherapists, Occupational Therapists, Nurses, and Support Workers, and works alongside Primary Care and Partners to provide Urgent Care Response, Frailty Hub, as well as short-term reablement/rehabilitation programmes. Responsible for holistic/nurse-specific assessing, developing, implementing, and evaluating patient care, ensuring high standards of care to patients. Ensure staff maintain detailed, accurate, and confidential patient records, adhering to Trust policies. Prioritise and delegate work using staff and time appropriately, referring patients to other agencies as necessary. Provide clinical advice and support to other staff. Maintain positive communication links between Primary Care, Secondary Care, Social Services, and Voluntary Agencies. The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy. Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required. Education, Qualifications and Training

Registered Adult Nurse with NMC registration. Registered and up-to-date mentor. Experience and Knowledge Required

Post-registration experience of working within community or hospital services. Skills and Attributes

Ability to work effectively within an integrated team. Ability to prioritise work and manage own work time effectively. Able to plan, deliver and evaluate programmes of care and promotion of health and well-being. Ability to work independently. Understanding of clinical governance. Excellent interpersonal and communication skills. Demonstrate ability to teach others. Aptitude and Personal Qualities
