About the Role:
Assess, plan, implement and evaluate individualised care using evidence based guidelines.
Work collaboratively with the inter-disciplinary team to deliver high quality care.
Maintain all records in line with corporate policies and procedures and NMC guidelines.
Carry out all clinical care in line with company policies and procedures.
Ensure the care team provides all care in line with the inter-professional evidence based care plan.
Where appropriate, be responsible for the management of patient care during the shift.

Our service provides 26 residential beds with opportunities for professional growth and development across complex disability, high dependency care, and slow stream rehabilitation.
