We are seeking a dedicated Rehabilitation Support Worker to join our clients team in Somerset and the surrounding areas; therefore, access to your own transport is essential. Our client is committed to helping customers lead independent lifestyles post-diagnosis or health condition. As a Rehabilitation Care Assistant, you will play a pivotal role in assisting customers with their rehabilitation needs in the comfort of their own homes. YOU MUST HAVE YOUR OWN TRANSPORT AND WE DO NOT OFFER SPONSORSHIP FOR THIS POSITION Key Responsibilities: Provide safe, reliable, and compassionate support tailored to each customer's individual rehabilitation programme. Collaborate with the care team, hospital discharge team, and occupational therapist to assess and meet the rehabilitation needs of customers. Respect and promote the dignity of each customer, ensuring care is delivered with informed consent. Monitor and report customer progress towards achieving rehabilitation goals in a professional and timely manner. Ensure clear communication with cluster groups of fellow professionals to aid customers' rehabilitation. Duties: Work under the guidance of the occupational therapist to design and enhance person-centred rehabilitation plans. Assist customers with personal care, medication management, meal planning and preparation, housework, and more. Safely use aids and equipment while respecting customers' dignity. Engage customers in social, physical, and mental activities to promote well-being and independence. Record and report all relevant customer information, changes in condition, emergencies, and safeguarding matters. Maintain confidentiality of customer and family information. Salary & Benefits: Competitive annual salary of £26,000. Travel allowance of 35p per mile. Full training provided. Overtime opportunities available. Essential Qualities: Compassionate, with good communication skills. Ability to work independently and travel across Somerset and surrounding areas. Clear understanding of when to seek help and advice to ensure customer safety and well-being.
