The Cheshire and Merseyside Rehabilitation Network (CMRN) is looking for a proactive and suitably experienced RGN or AHP to join our dynamic team as a Rehabilitation Coordinator. You will join a well-established Rehabilitation Coordination Service Team and be responsible for a designated caseload of highly complex rehabilitation patients, working closely with the wider specialist rehabilitation multi-disciplinary team, to provide a comprehensive, evidence-based, patient-centred service. You will play a key role in supporting patient’s discharge, working closely with external agencies including health care, social care, third sector and voluntary providers.

With a clinical background, this role as a Rehabilitation Coordinator will provide you with the opportunity to further expand knowledge and skills outside of a direct clinically facing role, including enhanced communication, organisational and planning skills while managing your own caseload flexibly and autonomously. You will still retain the role as a key member of the rehabilitation multidisciplinary team. You will be required to act as part of the Single Point of Contact for referrals into the network and undertake assessment, providing expert clinical advice on the most appropriate rehabilitation pathway.

The CMRN is a unique collaborative integrated pathway delivered across multiple organisations, hyper-acute, spoke, extended and community services, providing specialist rehabilitation to patients who have suffered traumatic injury or illness dependent on need and not diagnosis.

The post will be primarily based at The Walton Centre, with a requirement to work across and support our other network units based on Broadgreen, St Helens or Wirral sites.

As a Rehabilitation Coordinator, you will play a pivotal role within the multi-disciplinary team, acting as a key worker to a designated caseload and advocate for patients, families and carers to navigate them through their rehabilitation pathway.

You will provide support for patients with highly complex rehabilitation needs and their families/carers, and you will have excellent negotiation and persuasive motivational skills to manage complex situations and difficult communications.

Excellent communication skills are essential as liaison with referrers and working closely with the multidisciplinary team including Consultants, Nurses and Allied Health Professionals is an integral part of the role.

The post holder will be required to act as part of the Single Point of Contact for referrals into the Rehabilitation Network and undertake assessment, providing expert clinical advice on the most appropriate rehabilitation pathway.
