7529 About the Role: Lead shifts and coordinate rehabilitation programs for individuals Safely administer, prompt, or assist with medication management. Provide essential mobility support and personal care, following the care plan and risk assessments Collaborate with the therapy team and other professionals to create and update care plans. Maintain accurate written and electronic records Facilitate smooth admissions and discharges, promoting independence Create a positive, stimulating environment for our residents. The goal is to make sure residents enjoy fulfilling and dignified lives – and you will play a big part in that. Minimum Requirements: Hold a Level 3 qualification in Health & Social Care (or equivalent qualification, or experience) or be willing to work towards it. Have experience of administering medication and supporting individuals by following therapy-led rehabilitation goals Practical, positive, and ready to work in a team while managing your workload. Ready to make a meaningful impact with your care. Flexible for shift work, including weekends and sleep-in duties when required.
