Job Overview We are seeking a dedicated and compassionate RN or LCSW to join our dynamic team as a Clinical Care Specialist in a remote capacity. This role is pivotal in enhancing the quality of life for Medicaid patients across the nation, and you will serve as the clinical authority, collaborating with our operations team and healthcare partners to facilitate effective care coordination and optimize patient outcomes. Key Responsibilities Conduct comprehensive assessments to identify patients' complete clinical requirements. Analyze health insurance benefit plans to ascertain coverage possibilities. Partner with hospitals, providers, and community resources to ensure coordinated care following discharge. Develop treatment strategies, engaging patients and their families to identify the most suitable care alternatives. Maintain precise and thorough case documentation, detailing all interactions with both internal and external teams. Establish and nurture strong, trusting relationships with medical teams and essential stakeholders. Advocate for patients to guarantee access to vital resources and services. Navigate complex clinical scenarios swiftly, ensuring resolution within a 1-2 business day timeframe. Participate in healthcare team meetings as required to address patient care issues and eliminate barriers to care. Approach challenges with a proactive problem-solving attitude and provide effective solutions. Required Skills Exceptional communication and relationship-building skills. Strong problem-solving capabilities. Proficient in managing clinically intricate situations with promptness. Comfortable working remotely while maintaining active engagement with team members. Qualifications Education: Bachelor's degree in Nursing or Social Work with an active, unencumbered Massachusetts license. Experience: Minimum of 3 years in case management, managed care, or care coordination.
