Do you have a sound knowledge of respiratory conditions and how integrated working can be used to support this client group? Are you passionate about empowering your patients? More emphasis on community-based care means there's never been a better time to join our team. Having been awarded 'Outstanding' in a recent CQC inspection, we are Kent's leading provider of NHS services in the community and we are looking for a highly motivated respiratory specialist practitioner to join our specialist respiratory team in East Kent. If you successfully secure this post you would work alongside an established multi-disciplinary team and in addition hold a caseload of complex patients with a range of respiratory conditions that require specialist input. You would autonomously manage the assessment, planning, delivery and evaluation of patient care and utilise a whole systems approach when supporting admission avoidance for this client group. Main duties of the job

This will include putting in place management plans for patients who have complex respiratory conditions. In return for your hard work we can offer you a flexible working environment; personal development and a comprehensive corporate and local induction programme. As a trust, we strongly believe in making sure you have the right support, resources and training to enjoy your job and we provide a range of services to keep you in the best of health. We currently have bases in four localities across East Kent and whilst the successful applicant will be based in the Canterbury locality, they will be expected to be flexible in their approach and travel to other areas within East Kent when required. The post is for 30 hours per week based in our Canterbury area and we will consider a band 6 or a band 5 development post looking to achieve a band 6 role who demonstrates a passion to work within the respiratory field. We as a trust will support with learning, development and progression. Additional Working Pattern: 30 hrs a week - pattern to be decided - no weekends or Bank Hols. About us

Rated 'outstanding' by the CQC, we've got a passion for delivering high standards of patient care and excellent services to improve the health of our communities. To do this we need outstanding people who share our values: compassionate, aspirational, responsive, and excellent. We know a commitment to equity, diversity and inclusion leads to better patient care and a happier workforce. We pride ourselves in being diverse and welcome applications from people with varied backgrounds, perspectives and experiences. Job responsibilities

The job description and person specification gives you all the information you need about this role. Please look carefully at the criteria in the person specification and tell us what you have done that shows you meet this in your application.
