We're seeking an experienced and compassionate Registered Nurse to join our team as a Home Health Supervisor in Greenwich, CT. In this pivotal role, you'll lead a dedicated team of clinicians, manage patient care, and drive quality assurance initiatives. Key Responsibilities

Oversee daily patient care operations and clinician scheduling Manage referral intakes and third-party payer authorizations Conduct clinical record reviews and participate in monthly QA activities Coordinate interdisciplinary care teams and case conferences Perform OASIS reviews and ensure timely submission of visit notes Participate in on-call rotation and provide direct patient care as needed Drive continuous improvement in CMS 5-star ratings Cultivate relationships with referral sources and community partners Qualifications

Active RN license in Connecticut Minimum 2 years nursing experience in public health, home health, or hospital settings 1+ year of supervisory experience in healthcare Strong leadership and communication skills Proficiency in EMR systems and Microsoft Office Suite Current CPR certification Clean criminal background check Compensation
